Thank you for your assistance us in making the documentary film London Rebel Dykes of the 1980s (LRD80s). We have identified that you are in one or more still photographs we would like to use in the film. Please can you complete this model permission form. We are contacting photographers seperately to gain their permission too.
We will not use any images that are have drug references, sexual or taken at Chain Reaction without specific permission - and will contact you individually in these instances with this form.
This form will allow you to do two things:
1) General permission
If you are happy for all your images (unless compromising, sexual, or taken at Chain Reactions) to be used, in bar scenes, crowd scenes, friendship groups, marches etc. please fill in section one.
2) Section 2: submit permissions for up to 9 seperate photographs. Completing the form should take 1 to 4 minutes depending on number of photographs. Just describe the photograph. This may be an image you have seen in the Rebel Dyke production office page, or it might be one we have shared with you or one that you have sent to us.
Only fields with an asterick are required, however other information would be useful. Please do fill in form for photographs that you would not like to be used in film/donated to archive. This is important to prevent errors!
It is crucially important that you use the photographs file name/number or clear description of photo to identify the photograph you are giving permissions for so that we can identify the photograph.
