Should be your standard UoB username, but if this differs leave a note below saying why.

Dataset
|
Should be one listed on the intranet page. For all datasets by a study please enter the study part followed by a wildcard '*'.

Resource
|
Is usually a server name such as "bluecrystal". Please complete a new row for each resource.
